CANADIAN CITIZEN
Primary Identification – BRING ONE
- Original Canadian Birth Certificate
- Original, Valid Canadian Passport
- Original Naturalization Certificate
- Original Canadian Certificate of Citizenship
Proof of Social Security
- Original Social Security Card
- W2 or 1099
- Letter from Social Security with full name and Social Security Number
Note: Proof of Social Security is not required if no Social Security number has been previously issued .
Proof of Name Change
(Necessary only if name is different than on primary identification.)
The name that appears on the primary document is the name we must use for the driver license or identification card. To change your name on the driver license or identification card, you must first change it on your Canadian passport.
Proof of Address* – BRING TWO
- Deed, mortgage statement, or lease agreement
- Utility, internet, phone bills
- Valid Florida vehicle/vessel registration or title
- Medical bill or health card listing address
- Home or vehicle insurance bill
- Banking, credit card, or investment statements
- Mail from Federal, State, County or City government agencies
- Mail from schools or colleges
- W-2 or 1099 form
- Letter from homeless shelter or transitional service provider verifying residence there
- Postmarked personal mail (may not list the recipient as “occupant”)
- Completed Certification of Address form (English, En Español)
* Commercial learner’s permit (CLP) and commercial driver license (CDL) applicants must provide specific proofs of address; see details below.

US PERMANENT RESIDENT

United States lawful permanent residents (LPRs), also known as ‘green card’ holders, are non-citizens who are lawfully authorized to live permanently within the United States.
Primary Identification – BRING ONE
- US Permanent Resident Card (Green Card, Form I-551)
- Valid foreign passport and proof from United States Citizenship and Immigration Services (USCIS), such as approved I-797 receipt, immigration judge’s order, or stamp in passport showing that asylum or refugee status is granted/approved
- Immigrant Visa or I-551 stamp in valid passport
Proof of Social Security
- Original Social Security Card
- W2 or 1099
- Pay stub with full Social Security Number

NON-IMMIGRANTS
Document Requirements – English
Document Requirements – en Español
Non-Immigrants (anyone who is not a U.S. Citizen or Permanent Resident) are entitled to a driver license or ID card for the period of time listed in their USCIS document(s), up to a maximum of four years.
There are many acceptable non-immigrant documents depending on an individual’s situation and immigration status. Please bring all immigration documentation to one of our locations for review and acceptance.
Primary Identification – BRING ONE
Valid identification and current USCIS immigration documents must be presented each time testing, identification card, or driver license issuance is requested. The listing below is not all inclusive; please bring all current immigration documents to your appointment for review and acceptance.
- Valid Employment Authorization Card (Form I-766)
- Expired employment authorization card and I-765 filed prior to employment card expiration
- Valid Canadian passport or original/certified Canadian birth certificate
- Valid foreign passport and Immigration Judge’s Order or Notice of Hearing
- Valid foreign passport and current I-94
- Valid foreign passport and current USCIS documents authorizing Parole or Refugee status
- Valid foreign passport and pending application for Asylum
Proof of Social Security

Non-immigrants with a name change must first update their name with USCIS and Social Security Administration (SSA) . An original or certified copy of your marriage certificate or court order must be provided along with proof of name change with USCIS. Please wait at least one business day after updating name with social security before applying for an updated license or ID card. Canadian Citizens will need to update their passport and present an original or certified copy of marriage certificate or court order to apply for name change on Florida DL/ID.

Note: Those applying for a license will be issued a temporary driving permit ( en español ) ( an kreyòl ). The permit should be carried with photo identification until the hard card comes in the mail. Please wait until the last week the permit is valid to return to our office. That should allow enough time for the license to be approved and mailed from the Florida Department of highway Safety and Motor Vehicles.
